My Pets is a free applications that runs on ThinkDB 2.0 or later.
It will allow you to keep track of all information on your pets, including their medical information.
The applications consists of 3 files (Pet, Vet & Medical history).
After installing them to your PDA, tap on Pet and enter the basic information about your pet.
You may then tap on the Medical button to enter medical information about your Pet.

If you are a Windows user and have downloaded the wme4palm.zip file:
Extract the files using a standard unzip utility. Note the location of the directory/folder the file is extracted to.
Launch Palm Desktop. Select the Install Tool icon.
Select the correct user name from the User list.
Click on "Add" and browse to the folder where you extracted the files in the previous steps.
Select the folder "wme4palm" and select all the files. Click "Open". Select "Done" on the Installer tool. Place the corresponding handheld in the cradle and press the HotSync button on the cradle.

Locate the Quick Install icon on your Windows desktop.
Drag and drop the wme4palm.zip file onto the Quick Install icon.
